ABSTRACT

Kampo as the main treatment method in intensive care has been rarely reported. The patient is a 25-year-old man presented with diarrhea and hypotension. Before the current admission, he developed brainstem death after allogeneic bone marrow transplantation for aplastic anemia at age 13, and underwent cystostomy and received mechanical ventilation, gastrostomy nutrition, and in-home medical care. He developed cardiogenic shock with anuria and pleural effusion, due to Pseudomonas aeruginosa urosepsis and was treated with infusion/transfusion and enteral medicine with his family's consent. Tongue was pale and enlarged, with thin white fur. The radial artery was not palpable. The carotid artery pulse was relaxed, large, and faint. He had good abdominal strength, subcutaneous edema, and massive ascites. These indicated yang collapse pattern. Chest X-ray revealed a massive pleural effusion. Shimbuto and goshajinkigan Kampo extract supplemented with powdered cinnamon bark (Cinnamomum cassia Blume) was administered for the cardiogenic shock. Ogikenchuto extract combined with furosemide and goreisan extract was administered to treat the pleural effusion. Shimbuto, goshajinkigan, and powdered cinnamon bark improved the heart rate and blood pressure. The pleural effusion remained after furosemide and goreisan treatment, however, the addition of ogikenchuto induced significant diuresis, and the pleural effusion consequently decreased. This case suggests the potential of Kampo, even in critical illness where modern medicine is generally prioritized. The pleural effusion decreased after adding astragalus root-containing extract, emphasizing the role of tonifying qi for draining fluid.

ABSTRACT

Two cases of mental disorder with the tender point at the left paraumbilical region two fingerbreadths away from the umbilicus were successfully treated with shimbuto. These cases appeared shimbuto symptoms corresponding to the provisions of the early yang stage pattern and the middle yin stage pattern in the classic textbook ‘Shanhanglun'. The pathogenesis of these cases of the patients who improved with shimbuto would be qi and fluid intimidating. The tender point at the left paraumbilical region by palpation is considered as ‘Huangshu KI 16' of kidney meridian in ‘WHO Standard Acupuncture Point Locations in the Western Pacific Region'. This tender point is a very useful diagnostic sign to prescribe for simbuto. It suggests that an acupuncture point on the meridian and collateral theory might appear the condition of three yin and three yang disease in ‘Shanhanglun' and be useful to choose Kampo medicine.

ABSTRACT

We report a patient, who had neuropathic pain after radiation, called “chronic post-radiation pain syndrome,” who was successfully treated by Shimbuto. The patient was a 83-year-old man, diagnosed with Stage IB non-small cell lung cancer of the left upper lobe. Although the lesion had a surgical indication, he selected radiotherapy, and stereotactic body radiotherapy was performed. A few months later, he experienced neuropathic pain in his anterolateral chest wall. Loxoprofen and acetaminophen had little effect; moreover, tramadol hydrochloride/acetaminophen combination tablets and pregabalin induced drowsiness and dizziness. He then wanted to take Kampo medicine (Japanese traditional medicine) and was observed to have susceptibility to gastrointestinal disorders and cold intolerance by Kampo diagnosis. We prescribed Shimbuto 5 g per day, along with pregabalin. After 2 months, the pain had almost disappeared and he could stop taking pregabalin. Shimbuto is usually prescribed to patients at a risk of gastrointestinal disorders, pain, numbness, and other symptoms induced by cold intolerance. Shimbuto includes the extract of processed aconite root, which is effective for pain and numbness; therefore, it might be a good option for treating neuropathic pain when we have difficulties with Western medicine.

ABSTRACT

We report three closely watched cases of intractable Meniere's disease in the elderly that were successfully treated with Kampo medicine. Case 1 is a 75-year-old female. At the time of the 4th Meniere's attack, we gave her western medicines such as isosorbide, but she had a significant hearing loss. Then, we administered hachimijiogan extract granules, and her hearing level improved and she had been free of Meniere's attack for 23 months. Case 2 is a 78-year-old male. We diagnosed as bilateral Meniere's disease and treated him with western medicines such as isosorbide. In September of year X, his hearing level in the left ear worsened despite the treatment, so we prescribed hachimijiogan extract granules. After the treatment, we found the patient's hearing improved and nystagmus decreased gradually. He has also been free of Meniere's attack for 9 months. Case 3 is a 70-year-old male. He had repeated episodes of vertigo in spite of the administration of western medicines. After we prescribed shimbuto extract granules, vertigo attack and nystagmus disappeared. He has been relieved from Meniere's attack for 8 months. These cases indicate that Kampo therapy is one of the useful therapeutic options for intractable Meniere's disease in the elderly.

ABSTRACT

We treated dry eyes and eyestrains successfully with electrothermo-acupuncture and Kampo medicine. The case was a 41-year-old woman who was treated with a few eye drops. She came to our hospital to receive Kampo medicine because her photophobia and eye pain got worse. After taking shimbuto, her complaint got better with improving physical fatigue. Electrothermo-acupuncture was effective for photophobia and eye pain promptly and remarkably. After we added kihito to her, she did not need to use eye drops with improving bloody urine and sleeplessness. Photophobia and eye-ache are thought to be related to the trigeminal nerve system. In this case, we consider that the point of application of electrothermo-acupuncture is the trigeminal nerve system. At the same time, “rikan” and “hikyo” might be involved in this case with the general malaise of eye.

ABSTRACT

<p>We describe two cases of cardiac failure in elderly patients successfully treated with shimbuto. Case 1 was that of an 84-year-old man with severe aortic stenosis and liver cancer. His cardiac failure led to repeated hospitalization. After taking shimbuto, his pleural effusion decreased and cardio thoracic ratio improved. Case 2 was that of an 84-year-old man who was hospitalized because of cardiac failure and aspiration pneumonia after undergoing an operation for lung cancer. After taking shimbuto, his urine volume increased and cardiac failure improved. No notable adverse events were observed in any of the abovementioned cases. It is important that kanzo is not included in shimbuto to demonstrate <i>risui </i>action, hence the use of other “ho” formulations should be avoided. These cases suggest that shimbuto is indicated for elderly patients with cardiac failure under terminal care.</p>

ABSTRACT

A 48-year-old woman started to have dizziness after experiencing an earthquake. Her clinical signs improved with the use of shimbuto that was prescribed based on her symptom of yin-syndrome and hypofunction. At the same time, there was a decrease in reading errors with her finger, in a vein authentication device. One of the common reading errors that the security system makes is due to a change in blood flow due to vasoconstriction caused by low temperature. It is speculated that the use of a warming formula increased peripheral blood circulation, which contributed to the improvement of vein authentication.

ABSTRACT

We describe three cases of calf cramps successfully treated with shimbuto. Case 1 was a 74-year-old female who suffered from calf cramps and knee pain caused by gonarthrosis. She was treated with boiohito, but did not respond. When she complained of diarrhea, shimbuto was substituted for boiohito. After taking shimbuto, her knee pain improved, and her cramps disappeared. Case 2 was a 77-year-old female who suffered from leg edema and calf cramps. Tokishakuyakusan was prescribed, but she complained of an upset stomach. Tokishakuyakusan was replaced with shimbuto, and her cramps improved together with a decrease in the coldness and edema in her legs. Case 3 was a 79-year-old female who took kososan for uneasiness and restlessness after the death of her husband. She had edema and coldness in her legs, and suffered from calf cramps during the daytime and early in the morning. Shimbuto was added to kososan, and her coldness improved ; subsequently the leg edema diminished, and finally her cramps disappeared.<br>These cases suggest that shimbuto is indicated for patients with calf cramps, who report internal coldness caused by kidney deficiency with gastrointestinal dysfunction.

ABSTRACT

In this report, the author describes a case of recurrent ileus (intestinal obstruction) successfully treated with shimbuto. The patient was a 58-year-old man having been undergoing maintenance hemodialysis (HD) for 8 years. He had a history of laparotomy in his childhood. He had been admitted to the hospital frequently for recurrent ileus over the past 2 years. Although he was prescribed pantothenic acid, dinoprost, daikenchuto, and hangeshashinto, these medicines did not work. Eventually, he was transferred to our hospital. On admission, he suffered from abdominal pain and vomiting. Abdominal radiographics showed small intestine gases. Ab dominal CT, PET-CT, gastrointestinal endoscopy, colonoscopy, enteroscopy, and enterography through an ileus tube were performed to examine the cause of obstruction, however, no mass nor volvulus was observed. He recovered from the nausea and pain subsequent to ileus after taking shimbuto. At present, he has been pre scribed shimbuto without any recurrence of ileus for over 10 months. Patients undergoing maintenance HD are generally in a state of chronic water metabolism abnormality (<i>suitai</i>), abnormal defecation, and coldness in the extremities. Thus, the use of shimbuto for ileus might be recommended in patients undergoing HD.

ABSTRACT

It is assumed that keishikaryojutsubuto was first described in the Ho-ki medical text, and it has been used to target neuralgia and the arthritic pain. We experienced cases of Crohn's disease, endometriosis, post-operative rectal carcinoma, acute gastroenteritis and Meniere's disease successfully treated with keishikaryojutsubuto. When this formulation is administered, it might be important to keep in mind the definition of prescriptions for keishikashakuyakuto and shimbuto, or ryokeijutsukanto, as described in the Kokan-igaku text. In addition, this formulation might be applicable to various other diseases.

ABSTRACT

We report here the case of a 40-year-old female patient who presented at our outpatient clinic complaining of headache related to multiple systems, including the bowels, the viscera and meridians. She had two types of headache with different characteristics, which were enhancing factors and alleviating factors : (1) Squeezing pain in the posterior occipital area related to the kidney meridian, and (2) Pain in the lateral occipital area and areas deep beneath the eyes and ears, related to the large intestine, the stomach, and the triple energizer meridian and gallbladder meridians. Following a traditional logic-based therapy, we diagnosed her with edema due to a kidney yang deficiency and a liver-qi ascending counter flow. Both types of headache were alleviated and resolved, after shimbuto was administered to clear the channels by both draining the water due to warming yang and soothing the liver, and sending down abnormally ascending liver qi. The original cause of the disease was the kidney yang deficiency, and the immediate cause was the liver qi ascending counter flow. We believe that these affected each other fundamentally and incidentally because the lateral occipital pain, which was the incidental symptom, was resolved with shimbuto.

ABSTRACT

We report a case of 87-year-old female with somatoform autonomic dysfunction whose complaint of oppression ranging from the left chest to the epigastrium was successfully treated with shimbuto, in whom neither antidepressants nor anxiolytic drugs had been effective. Since weak abdominal power, bilateral kyokyokuman and pulsation in the upper navel region were observed, saikokeishikankyoto was initiated, but satisfactory effect was not obtained. Thus, we re-estimated her conditions and judged as the stage of Shao yin based on symptoms such as bedridden tendency, coldness of the limbs and indigestible diarrhea, although a weak and surface pulse was noted. Shimbuto was then administered, and various symptoms including chest oppression were improved rapidly and dramatically. We speculated that her chest oppression might occur due to kiutsu (depression of ki) secondary to suitai (accumulation of sui) rather than kigyaku (regurgitation of ki), and that the improvement of suitai by shimbuto might result in the disappearance of kiutsu. Although shimbuto is not popularly used for the treatment of somatoform disorder having chest symptoms, our case suggests that there should be a certain case in which shimbuto is so effective. Also it is suggested that shimbuto might be applicable even when deep pulse or excessive strain of abdominal muscles is not necessarily observed.

ABSTRACT

We experienced 2 cases of allergic rhinitis successfully treated with Shimbuto. Shoseiryuto, Goreisan and Ryokankyomishingeninto did not have any favorable effect on these cases. They had similar subjective symptoms, that is, a mass of nasal mucus, became cold easily and developed taste for citrus fruits. There were common objective symptoms, such as, fullness of the upper abdomen, coldness of the epigastrium, epigastric throbbing pulsation and a fluid sound in the stomach upon palpation. These findings suggest that Shimbu-to might be practicable for the patients who did not received the blessing of the standard Kampo therapy.

ABSTRACT

A case of subphrenic abscess after Hassab's operation successfully treated with the Kampo formula Shimbu-to is reported. A 43-year-old woman who had been suffering from cirrhosis of the liver and pancytopenia received an operation. Ten days following the operation, she developed a fever. The condition was diagnosed as postsplenectomic sepsis and steroid hormones were given. On day 35 following surgery, a subphrenic abscess was detected by CT scan and echography. The abscess was drained using echography, and several types of antibiotics given, however, these therapies were not effective. After administration of the Kampo formula Shimbu-to, the subphrenic abscess decreased.<br>Shimbu-to proved effective in the treatment of a subphrenic abscess that did not respond to Western medical drainage and antibiotic treatment methods.
